[BKARTS] Format mystery solved

For those of you who were curious - the formatting of the headers in the "on
behalf of.." style is a bug in Microsoft Outlook. Switching to Outlook
Express solved the problem. (Note to self - don't alert 1500 people about
something until you know what you're talking about - end note)
