We are increasingly being asked to loan books from our library collection, 
and our registrar asked me to post this query to Exlibris:

The registrar at the Amon Carter Museum would like to see samples of 
condition report forms for books lent for traveling exhibitions.  The 
registrars committee of AAM has plenty of examples for art objects but 
she has had trouble finding one for books.  If you have one that you
